“The scan was normal” — but you still don’t feel right

It is one of the most common stories after a car accident or a blow to the head: the emergency room rules out the dangerous things, the CT looks fine — and weeks later you are still foggy, slow, headachy, not yourself. Medicine has a name for this family of injuries: concussion, also called mild traumatic brain injury (TBI) — and its effects are functional, which is exactly why they are so often invisible on a structural scan. Structural imaging and functional measurement answer different questions. A scan shows what your brain looks like. The P300 is one of the most-studied signals in brain science — first described in 1965, and the subject of thousands of published studies in the decades since. It marks the moment your brain registers and classifies information, and its timing is measured in milliseconds. Slower P300 timing and slowed reaction time have been documented in published research across concussion, traumatic brain injury, aging, and cognitive decline.
